Welcome to Ulster County NY Connects, our online resource designed to give the residents of Ulster County access to a variety of information regarding Eldercare, Disabilty, Long Term Care and other Health Care Services, Community Based Services, Meals on Wheels, Food Pantries, Insurance information, as well as basic information about Medicare, Medicaid, and Prescription Drug coverage. |
Throughout the state, the New York State Office for Aging and the Department of
Health are working with local Offices for the Aging, Social Services and Health
Departments to empower individuals to make informed choices and to streamline access
to long-term care services.
Long-term care services refers to the wide range of in-home,
community-based and institutional services and programs that are designed to help the
elderly and individuals with disabilities of all ages.

In Ulster County, the Office for the Aging, the Department of Social Services and
the Department of Health have been working towards that goal for several years, and are
currently designing a proposal to ensure that our elderly and disabled will be able to
access the care they need with one central point of entry for all of the services they
require.
The vision of the program is to provide an accessible, coordinated and personcentered
long term care system that: supports self-determination and promotes personal
responsibility; provides services that meet consumer needs; provides quality care, and
provides efficiency and affordability. |
